Field Descriptions¶

• PLACE OF INCIDENT Field Details: A free format text field describing the scene.
  

• PLACE OF DEATH Field Details: Select from a drop down list with these options: INPATIENT, ER OUT/PT, DOA, NURSING HOME, RESIDENCE, OTHER
  

• PLACE OF DEATH (IF HEALTH CARE FACILITY) Field Details: Select from a drop down list from the FACILTY table.
  

• DATE ADMITTED - You can enter the date in format mm/dd/yyyy or select the calendar button and select a date.
  

• TIME ADMITTED - Enter the time of the incident as hh:mm or hh:mm:ss military time, if known.
  

• PLACE OF DEATH (OTHER THAN HEALTH CARE FACILITY) Field Details: Free form text where death occured if not in a medical facility. This would be true when RESIDENCE or OTHER were selected for PLACE OF DEATH.
  

• MUNICIPALITY HERE DEATH OCCURED Field Details: Select from a drop down list from the MUNICIPALITY table.
  

• DOES DECEDENT HAVE A DNR ON FILE Field Details: A check box selection.
  

• DNR DATE - You can enter the date in format mm/dd/yyyy or select the calendar button and select a date.
  

• LIGHTING CONDITION - Select the drop down description that matches the lighting condition at the scene.
  

• WEATHER - Select the drop down description that matches the weather at the scene.
  

• HVAC STATUS - Select the status of the HVAC.
  

• HVAC MODE - Select the mode of the HVAC, only becomes active when HVAC Status is set to ON.
  

• THERMOSTAT SETTING - Enter the temperature setting of the thermostat.
  

• OUTSIDE TEMPERATURE - Enter the outside temperature of the scene.
  

• INSIDE TEMPERATURE - Enter the inside temperature of the scene.
  

• ANIMALS/PETS - Select if any animals/pets found on the scene.
  

• CLEANLINESS - Select the drop description that matches the cleanliness of the scene.
  

• HOARDING - Select if the scene involves hoarding.
