To Add a Toxicology Specimen¶

1. Click the “New” button near the bottom of the screen.

2. Select where the specimen was taken.

3. Select when the specimen was taken.

4. Select the Status.

5. Select or type in the Pathologist that collected the specimen.

6. Select or type in the Technician that collected the specimen.

7. Select the Specimen that was collected.

8. Choose the Date the specimen was collected.

9. Click the “Save” button near the bottom of the screen to save the Toxicology specimen.

To Add a Chain of Custody¶

1. Select the Toxicology specimen and click the “Edit” button near the bottom of the screen.

2. Enter the Date the specimen changed custody.

3. Enter in who Relinquished the specimen.

4. Enter in who Received the specimen.

5. Enter in the Purpose of the specimen transfer.

6. Click the “Insert” button to save the record.

To Add a Test Performed¶

1.Select the Toxicology specimen and click the “Edit” button near the bottom of the screen.

2. Enter the Date the specimen was sent off for testing.

3. Select or type in the Test Type.

4. Select or type in the Facility used.

5. Enter the Date the test results were returned.

6. Click the green “+” symbol to the left the of row you have entered to save the record.

To Add a First Test Result¶

1. Select the Test you wish to add results to and click the edit “cd” icon.

2. Enter in the Substance Tested For.

3. Select the Detection Level.

4. Enter in the Amount.

5. Choose or type in the measurement Unit used.

6. Click the green “+” symbol to the left of the row you have entered to save the Test Results record.

7. If you had to edit the Test Performed row to add a new result click the blue “disk” icon to save the Test Performed record.

To Add a Additional Test Results¶

To add additional results you do not need to edit the Test Performed row. Follow steps 2-6 in the instructions above for the test you wish to add results to.

Field Descriptions¶

TOX TAKEN: Select where the specimen was taken.

TAKEN: Selet when the specimen was taken.

STATUS: Select the status of the specimen.

PATHOLOGIST: Select or enter the pathologist that took the specimen.

TECHNICIAN: Select or enter the technician that took the specimen.

SPECIMEN: Select the specimen taken.

DATE COLLECTED: Enter the date the specimen was taken.

SPECIMEN CHAIN OF CUSTODY: Track the chain of custody for the specimen. Only available once a specimen has been saved.

TESTS PERFORMED: Enter one or more tests performed on the specimen. Only available once a specimen has been saved.

RESULTS FOUND: Enter one or more results for each test performed. Only available after a test performed has been inserted. Entering the first result requires editing the tests performed row. Subsequential results do not require editing the test performed row.
