Field Descriptions¶

• LAST NAME Field Details: The last name.
  

• FIRST NAME Field Details: The first name.
  

• MIDDLE NAME Field Details: The middle name or middle initial
  

• ADDRESS 1 Field Details: A free form text field.
  

• ADDRESS 2 Field Details: A free form text field.
  

• CITY Field Details: A free form text field.
  

• STATE Field Details: Select from a drop down list of US states and Canadian Provinces.
  

• ZIP CODE Field Details: Select from a drop down list of US zip codes or Candian codes.
  

• PHONE Field Details: There are three available telephone entries. In the TYPE field the selection options are HOME, WORK, PAGER, OTHER from a drop down list. Enter the phone number including area code in the PHONE field. Enter and extension number in the EXT field.
  

• RELATIONSHIP Field Details: A free form text for the relationship of the person that is the next of kin of the individual.
  

• VERIFIED BY Field Details: Select from a dropdown list of available items located in the office table.
  

• VERIFIED WITH Field Details: A free form text field. An example could be a family member name, state BMV, etc.
  

• DATE VERIFIED Field Details: Enter or select a date from the calendar selection button.
  

• TIME VERIFIED Field Details: Enter in miltary time
  

• MARITAL STATUS Field Details: marital status is a selectable item from a dropdown list. The dropdown list includes MARRIED, NEVER MARRIED, WIDOWED, DIVORCED, SINGLE, SEPARATED.
  

Spouse Field Descriptions¶

• LAST NAME Field Details: The last name.
  

• FIRST NAME Field Details: The first name.
  

• MIDDLE NAME Field Details: The middle name or middle initial
  

• Maiden Name Field Details: A free form text field for maiden name of a female spouse.
  

Last Contact Field Descriptions¶

• DATE Field Details: Enter or select a date from the calendar selection button.
  

• TIME Field Details: Enter in miltary time
  

• TYPE Field Details: A free form text for the type of contact made. Example could be VISUAL, CONVERSATION ON PHONE, etc.
  

• NAME Field Details: A free form text for the full name of the person having the last contact with the individual.
  

• PHONE Field Details: There are three available telephone entries. In the TYPE field the selection options are HOME, WORK, PAGER, OTHER from a drop down list. Enter the phone number including area code in the PHONE field. Enter and extension number in the EXT field.
  

Family Field Descriptions¶

• NOTIFIED? Field Details: Select from a dropdown list of available items located in the office table. The selection options are YES or NO.
  

• BY Field Details: A free form text for the full name of the person having the last contact with the individual.
  

• DATE Field Details: Enter or select a date from the calendar selection button.
  

• TIME Field Details: Enter in miltary time
