FIELD DESCRIPTIONS

  • INCIDENT OCCURED DATE - You can enter the date in format mm/dd/yyyy or select the calendar button and select a date. The age at death will be calculated using this date.

  • INCIDENT OCCURED TIME - Enter the time of the incident as hh:mm or hh:mm:ss military time, if known. The age at death will be calculated using this time.

  • NOTIFIED DATE - You can enter the date in format mm/dd/yyyy or select the calendar button and select a date.

  • NOTIFIED TIME - Enter the time of the incident as hh:mm or hh:mm:ss military time, if known.

  • ARRIVED DATE - You can enter the date in format mm/dd/yyyy or select the calendar button and select a date. The age at death will be calculated using this date.

  • ARRIVED TIME - Enter the time of the incident as hh:mm or hh:mm:ss military time, if known. The age at death will be calculated using this time.

The COPY DATES button can be used to copy the ARRIVED date and time into the PRONOUNCED, FOUND, DATE OF DEATH and TIME OF DEATH

  • PRONOUNCED DATE - You can enter the date in format mm/dd/yyyy or select the calendar button and select a date. The age at death will be calculated using this date.

  • PRONOUNCED TIME - Enter the time of the incident as hh:mm or hh:mm:ss military time, if known. The age at death will be calculated using this time.

  • FOUND DATE - You can enter the date in format mm/dd/yyyy or select the calendar button and select a date. The age at death will be calculated using this date.

  • FOUND TIME - Enter the time of the incident as hh:mm or hh:mm:ss military time, if known. The age at death will be calculated using this time.

  • DATE OF DEATH UNKNOWN - This box can be checked if the date of death is unknown.

  • DATE OF DEATH - You can enter the date in format mm/dd/yyyy or select the calendar button and select a date. The age at death will be calculated using this date.

  • DATE OF DEATH (TYPE) - Select from the drop down list. The list included ACTUAL, BASED ON DATE FOUND, APPROXIMATELY.

  • REPORTED BY - Enter the name of the person reporting the incident.

  • (REPORTED) OF - Enter the name of the organization, if any, of the person reporting the incident.

  • DISPATCHER - Enter the name of the dispatcher and or badge number.

  • INCIDENT NO. - Enter the number assigned to the incedent.

IF INJURY

  • ACTUAL INJURY DATE - You can enter the date in format mm/dd/yyyy or select the calendar button and select a date.

  • INJURY DATE BETWEEN - This box can be checked if the date of death is between two dates.

  • DATE - You can enter the first between date in format mm/dd/yyyy or select the calendar button and select a date.

  • DATE - You can enter the second between date in format mm/dd/yyyy or select the calendar button and select a date.

  • TIME - You can enter the time in format hh:mm or hh:mm:ss military time.

  • ACTUAL INJURY DATE (TYPE) - Select from the drop down list. The list included ACTUAL,APPROXIMATELY, UNKNOWN.
