Field Descriptions¶

• INVESTIGATING CORONER/ME Field Details: Select the appropriate investigating Coroner or Medical Examiner from the drop down list.
  

• CERTIFICATE ISSUED BY Field Details: Select the appropriate name of the person who issued the death certificate from the drop down list.
  

• DATE CERTIFICATE SIGNED Field Details: Enter the appropriate date or select it by clicking on the calendar icon.
  

• REVISED CERTIFICATE Use the check box to indicate that the death certificate has been revised.
  

• CERTIFICATE DISPOSITION Field Details: Enter a text description of the death certificate disposion. Examples: blank, FINAL
  

• EXPENSES INCURRED Field Details: Enter expenses related to this case.
