Field Descriptions

Part I

  • IMMEDIATE CAUSE Field Details: Enter the primary cause of death by entering the first few characters of the description, a drop down list will allow selection of the actual cause. If a cause is not located in the table you may add a custom decsription which will be added to the case table. You may enter up to three additional causes of death.

    TIP: Key in the first two characters of the cause description and a list will be displayed for selection.


  • INTERVAL BETWEEN ONSET AND DEATH Field Details: Enter the interval for each cause of death entry or leave blank.

  • MANNER OF DEATH Field Details: Select the drop down description that indicates the correct manner of death.

Part II

  • OTHER SIGNIFICANT CONDITIONS Field Details: Enter other sigificant conditions related to the person by entering the first few characters of the description, a drop down list will allow selection of the actual cause. If a cause is not located in the table you may add a custom decsription which will be added to the case table. You may enter up to four other conditions.

  • INTERVAL BETWEEN ONSET AND DEATH Field Details: Enter the interval for each cause of death entry or leave blank.

  • MANNER OF DEATH Field Details: Select the drop down description that indicates the correct manner of death.

  • ALCOHOL PRESENT Field Details: Select a drop down description regarding alcohol.

  • BAC Field Details: Enter the BAC percentage level if tested. The system will automatically insert a decimal. Example: If "009" is entered, it will be edited to ".009"

  • CARBON MONOXIDE Field Details: Select a drop down description regarding carbon monoxide level. Enter the CO and include a decimal point where appropriate. EXAMPLE: .050

  • CO (level) Field Details: Enter the carbon monoxide percentage level if tested. The system will automatically insert a decimal. Example: If "50" is entered, it will be edited to ".50"

  • DRUG USE Field Details: Select a drop down description regarding whether drugs were detected.

  • SUBSTANCE Field Details: You can enter multiple substances in the text area. These can be selected for printing on the REPORTS menu.

  • PRONOUNCED BY Field Details: Select the person that prononced the death from the drop down list.

  • PERSON CERTIFYING CAUSE OF DEATH Field Details: Select the person certifying the cause of death from the drop down list.

  • TOBACCO USE Field Details: Select whether tobacco contributed to the death from a drop down list. Options are: YES, NO, UNKNOWN

  • PREGNANCY STATUS Field Details: If female, select prgnancy status from a drop down list. Options are: NOT PRGNANT, PREGNANT, (STATE SPECIFIC TERMS), UKNOWN

Medical Interventions

  • INTERVENTIONS: Select one or more checkboxes on interventions performed.

  • INTERVENTIONS PERFORMED BY: Choose one or more for who performed an intervention.

  • CPR INITIATED: Enter the date and time CPR was initiated if performed, unavailable unless CPR Intervention selected.

  • CPR TERMINATED: Enter the date and time CPR was terminated if performed, unavailable unless CPR Intervention selected.

  • NALOXONE ADMINISTERED: Select whether Naloxone was administered.

  • BYSTANDER DOSE(S): Enter the name and number of doses of Naloxone administered by bystander, unavailable unless Naloxone administered is set to Yes.

  • FIRST RESPONDER DOSE(S): Enter the name and number of doses of Naloxone administered by first responder, unavailable unless Naloxone administered is set to Yes.

  • REASON INTERVENTION TERMINATED: Select the drop down description for the reason interventions were terminated.

  • REASON INTERVENTION TERMINATED (specify): Available if Other is selected for Reason Interevention Terminated.

  • TERMINATED BY: Enter who terminated interventions.

  • INTERVENTION NOTES: Enter any notes about interventions performed or not performed.
